We offer free Medicare 101 educational classes each month at our office. Understanding Medicare can be overwhelming. At our Medicare 101 class, one of our agents will help demystify: The A, B, C (and D) of Medicare, Supplementing Medicare, Choosing a Drug Plan, and When and How to Enroll.
Already on Medicare but want to make a change to your current coverage?
The Annual Enrollment Period is every year, from October 15th to December 7th. We are happy to review your medical and prescription coverage to see if a plan change is right for you. As this time of year approaches, we will be holding Medicare 101 classes and Advantage Plan presentations that will give an overview of the upcoming year’s benefit plans. Check back here for more information around October 1st.

Scope of Appointment Form
In order to schedule an appointment, Medicare requires that you complete a Scope of Appointment form 48 hours prior to your appointment. This document is for your protection and states that we will only be discussing Medicare options with you, and no other types of insurance.
New Client Intake Form
Prior to your appointment, please complete our Client Intake form that provides us with general information about you, such as your name, address, phone number, date of birth, etc. This ensures we can allocate as much of your appointment as possible to answering any questions you may have. If you're an existing client, you do not need to complete this form.
Prescription Analysis Form (Optional)
If you would like us to do a comprehensive analysis of your out-of-pocket costs for your medications, along with the drug plans available to you, we are happy to assist! Please include all medications you take (except over-the-counter products) along with the dosage, refill frequency, brand name or generic, and your pharmacy. If you use insulin, please note the vial units/ml and how many you use per month. If you use an inhaler, please bring it to your appointment. This form is completely optional.
What You Need For Your Appointment
- Medicare Card showing ID#, Part A and Part B Effective Dates, if applicable.
- Letter of creditable coverage (proof of employer coverage), if applicable.
